We review the family’s priorities, current supports, and practical needs. The result is a written roadmap with clear next steps and responsibilities.

Hospital-to-home transitions

We help organize the transition from a hospital or rehabilitation stay to home. That can include follow-up appointments, equipment, community services, and communication with people the family authorizes.

Finding the right resources

We help families identify and compare home care agencies, adult day programs, meals, transportation, and senior living options. Choices remain with the family.

Appointment support

We help prepare questions and organize follow-up tasks. When agreed in scope, a coordinator may attend and document the discussion for the family.

Benefits navigation

We provide plain-language information about public benefit programs and community resources. We do not sell, recommend, or enroll anyone in Medicare plans, and we do not give legal or financial advice.

Long-distance family support

We help families living elsewhere stay informed through an agreed update rhythm. With permission, we coordinate authorized communication and provide written updates.

We do not provide medical care, nursing, or hands-on personal care.

We are not an emergency service. In an emergency, call 911.

We do not bill insurance or enroll anyone in Medicare or Medicaid programs.
